Gastroduodenal intussusception secondary to pedunculated gastric carcinoma

Abstract

A dult intussusception is different in its entity than in children. 1-3 It is caused by a definable structural lesion mainly in the intestines.2-4 Rarely, gastroduodenal intussusception occurs as a condition that is secondary to a mobile leading gastric tumor that prolapses into the duodenum.5-10 Such a prolapsing phenomenon is observed in both benign and malignant cases. A diagnosis of adult intussusception can be made reliably with noninvasive imaging techniques such as sonography, computed tomography and magnetic resonance imaging. However, sonography has not been well described for diagnosis of gastroduodenal intussusception in adults. 6 We report the case of an adult patient who had intermittent symptoms of pyloric stenosis in whom sonographic examination revealed gastroduodenal intussusception secondary to a mobile leading gastric tumor that prolapsed into the duodenum. Histopathologic examination of the resected tumor revealed a pedunculated, well-differentiated tubular adenocarcinoma in the antrum of the stomach. This report describes the sonographic diagnosis of adult intussusception due to gastric carcinoma, suggesting the value of sonography in the differential diagnosis of intermittent symptoms of pyloric stenosis in adults.
