A case of rhegmatogenous retinal detachment in the convalescent stage of Vogt-Koyanagi-Harada disease

Background: We report a case of rhegmatogenous retinal detachment (RRD) in the convalescent stage of Vogt-Koyanagi-Harada (VKH) disease. Case Report: A 56-year-old female visited a local doctor because of floaters in her left eye. RRD was diagnosed and the patient was referred to Keio University Hospital. She had a history of VKH disease diagnosed 3 years previously on the basis of headache and bilateral uveitis and treated with pulse corticosteroid therapy. At her initial visit to our clinic her corrected visual acuities were 1.0 in both eyes with no ocular inflammation. Funduscopic examination revealed bilateral sunset-glow (yuyake) fundi and retinal detachment with a flap tear in her left eye. After cryotherapy around the break, a local buckle was placed. The retina reattached successfully without any recurrence of VKH disease. Conclusion: A conventional scleral buckling procedure was as successful as in uncomplicated cases in treating retinal detachment complicated by VKH disease with sunset-glow fundi and decreased retinal pigment epithelium function in the convalescent stage.
