We reviewed the results of 128 penetrating keratoplasties performed at Keio University Medical Center in 122 eyes of 116 patients between 1994 and 1996. The cases included 41 operations for leukoma, 27 operations for a failed graft, 21 operations for keratoconus, 20 operations for bullous keratopathy, 11 operations for corneal dystrophy, and 8 operations for other disorders. Each year, the number of cases with bullous keratopathy decreased whereas the number of those with a failed graft increased. A triple procedure was performed in 41 (32.0%) of the 128 cases. The graft remained clear at 1- year follow-up in 87.5% of cases. Allograft rejection occurred in 26.6% of cases and glaucoma occurred in 10.2%. Postoperatively, 54 (42.2%) of the 128 cases had visual acuity of 0.1 or better and 115 (89.8%) of the 128 cases had 2 or more lines improvement in visual acuity.
