Slit-lamp photographs are usually used to record signs of disorders in the anterior segment. However, it is sometimes difficult to record all changes in the cornea on one photograph. We therefore designed a chart to document findings in eyes with corneal disorders. On this "corneal chart", findings are color-coded, for easy recording and interpreting of results. The corneal chart consists of 3 parts: an overview of the cornea, a cross-sectional view of the cornea, and a view of the entire anterior segment, including the lid and conjunctiva. Each level of the cornea is assigned a color code so that changes in different levels of the cornea can be distinguished easily. We have so far used the corneal chart to document changes in eye with corneal disorders and eyes that have undergone corneal surgery. We found that the corneal chart is useful not only for evaluating disorders in the anterior segment but also for understanding the pathophysiology of corneal disorders.
