The aim of the high-dose methotrexate (MTX) chemotherapy followed by rescue by infusion of leucovorin (LV) is to enhance the effects of the MTX on luekemic cells without increasing its toxicity to normal leukocyte cells. Physiological aspects of the high-dose MTX therapy combined with the rescue agent LV are investigated via the modelling and control systems approach to attain careful administration of MTX and LV. A compartmental model is given to describe the pharmacokinetics of administered MTX and LV, the cell kinetics of normal and leukemic cells and the cell-kill and cell-rescue effects. Based on this model, the optimal control scheme for infusing the rescue agent LV is given in a closed-loop form using the monitored cell number of normal leukocyte so as to rescue the normal cells selectively from MTX toxicity. The optimal continuous therapy of MTX is also discussed on a standpoint of closed-loop control, which can be implemented by monitoring the leukemic cell number in blood.
